If you are member of the National Rifle Association, please give your NRA number.

If you are, or have been, a member of any other Club, Association or Society connected with shooting or firearms, please list them with dates of joining and leaving.




Have you ever been refused membership to a Shooting Club? If so, please give details.




Have you ever had an application for a Firearm Certificate refused or a certificate revoked. If Yes, please enter further details below or use Additional Information.

Attach Key Documents

Please attach a passport size photo.

1 - Current UK passport or EU/other nationalities passport 2 -Passports of non-EU nationals, containing UK stamps, a visa or a UK residence permit showing the immigration status of the holder in the UK 3 - A current UK full or European photo-card driving licence or provisional photo-card driving licence. 4 - A national ID card and/or other valid documentation relating to immigration status and permission of work.

1 - Recent utility bill (e.g. gas, electricity or landline) within the last 3 months. Mobile phone bills are not acceptable. 2 - Local authority tax (e.g. Council Tax) bill valid for the current year. 3 - HM Revenue and Customs tax notification(i.e. tax assessment, statement of account, notice of coding) a P45 or P60 is not acceptable. 4 - Financial statement (e.g. bank, building society, credit card or credit union statement) containing current address within the last 3 months. 5 - Most recent mortgage statement from a recognised lender within the last 12 months. 6 - Current benefit book or card or original notification letter from DWP confirming the rights to benefit (e.g. child allowance, pension) within the last 12 months. 7 - Current local council rent card or tenancy agreement. 8 - UK full or provisional photo-card driving licene (can only be used once for either proof of identification or residence). 9 - Confirmation from an electoral register search that a person of that name lives at the claimed address within the last 3 months.
References
Please give the names and permanent residential addresses of 2 referees. These cannot be close family. One can be a friend of at least three years and the other should be someone more formal e.g., a work colleague, Both referees must have UK addresses. If you are currently, or have within the last 2 years been a member of a shooting club, one of the referees MUST be the Secretary of that club.
Referee One
Please give the names and permanent residential addresses of 2 referees. These cannot be close family. One can be a friend of at least three years and the other should be someone more formal e.g., a work colleague. Both referees must have UK addresses.







Referee Two
If you are currently, or have within the last 2 years been a member of a shooting club, one of the referees MUST be the Secretary of that club. Please supply, email and phone number for each referee.
